In order for medication to be administered in the school setting:
A Medication Order must be completed and signed by the physician and parent.
Medications orders are valid for the current school year only and must be renewed at the start each school year.
Medication orders are required for both perscriptive and over-the-counter medications.
All medications must be received at school in their original packaging with a pharmacy affixed label showing the child’s name, medication, dosage and administration instructions. For an over-the-counter medication please provide the medication in its original sealed packaging, do not send open bottles.
For nebulizing medication, tubing and mask or mouthpiece must be provided.
Elementary students are not allowed to self-carry or self-medicate.
