Our school belongs to the Middlesex Middle School Athletic League which requires that we adhere to the Massachusetts Interscholastic Athletic Association's policies related to student eligibility requirements for participating in interscholastic sports. Please visit www.miaa.net for more information.
The Massachusetts Department of Public Health (MA 105 CMR 201.000) Head Injuries and Concussions in Extracurricular Activities) Regulation requires all public middle and high schools to develop a policy to provide standardized procedures to prevent and manage head injuries.
Per this regulation, certain requirements are mandated for students at Curtis Middle School who plan to participate in/try out for any extracurricular athletic program.
Athletic fee: $195 ($95 if it is your second sport of the year). Check should be made payable to the Town of Sudbury.
Completed within 13 months of an entire sports season.
If a student’s 13-month anniversary of their most recent physical occurs DURING the season, they must have a new physical exam in order to remain eligible to play. Your child must have this uploaded into Family ID to be eligible to try out. You will receive an email generated from Family ID when your child's physical is close to expiring.
This must be completed PRIOR to EACH sports season.
Forms are all on Family ID to complete.
This Concussion training course must be completed ANNUALLY by parents and students.
Acknowledgment of completion is on Arbiter Sports; you no longer need to turn in the certificate to the Health Office.
*Students will NOT participate in a sports tryout until all forms on Arbiter Sports are completed.
