Health Office Forms:

The school nurse will administer Tylenol and ibuprofen as needed if you have checked the consent box in the health section of the online registration. For all other medications please complete the medication permission form found below:

• Medication Permission 

• K-6 Physical Exam Form (required before preschool and encouraged K-6)

• 7-12 Physical Exam and Concussion Form (required annually for junior high and high school sports) *New form as of May 2023.  Physicals completed on the old form prior to July 1, 2023 will still be honored for the 23-24 school year. 

• Dental Form (required before kindergarten and 9th grade)

• Vision Form (required before kindergarten and again before third grade)

• Diet Modification Form

• Immunization Certificate (required for all students)

• Immunization Exemption (medical)

• Immunization Exemption (religious)

• Authorization for Medical Procedure

• Asthma Action plan

• Food Allergy Action plan

• Seizure Action plan

• Self-Carry consent for epinephrine pen and inhaler
