PLEASE CAREFULLY READ THE FOLLOWING!
BCCHS now requires all athletes to fill out ALL paperwork via online (athletic clearance). However, the athletes are still required to turn in a hard copy of their PHYSICAL to the coach and nurse. Please click on the following links below to register in the system (athletic clearance) and fill out all required forms to be eligible to try out! Once completed, please print out a copy of the PHYSICAL form down below, and visit your doctor.

Parent Game Day Release
According to BCCHS rules, all cheerleaders must ride the bus TO and FROM the event with their teammates and coaches. The following form allows a student do be released from the game and get picked up by their parents/guardians. It must be signed and given to the coach prior to the game/event.
