 for the 2024-2025 School Year

 

Welcome Back to St. Joan of Arc School  for another school year!! If you are new - Welcome!!

Listed below are health forms that are required by grade level for Re-Registration or Registration.

All New Students and Those Entering PreK-3 and PreK-4: 

A Completed MSDE Record of Physical Examination Form

A Completed MSDE Immunization Certificate (896)

A Dental Exam Form

A Completed School Health Information and Consent Form for the 2024-25 Schoo Year:  that is Signed by Parents

A MSDE Medication Administration Authorization Form (If Applicable) This form is used for any prescription medications that your child may need at school. This form is also used to give any Over-The-Counter Medications such as Acetaminophen or Ibuprofen. Medication should be sent in for your child in a labeled prescription container or small OTC Medication Bottle.


Kindergarten Students:

A Completed MSDE Record of Physical Examination Form

A Completed MSDE Immunization Certificate (896 )

Grades One and Grade Two:

A Completed School Health Information and Consent Form for the 2024-25 School Year - that is Signed by Parents

Grade Three:

Grade Six (Entering Middle School):

A Completed MSDE Immunization Certificate (896 )

A Dental Exam Form

A Completed School Health Information and Consent Form for the 2024-25 Schoo Year:  that is Signed by Parents

Grade Seven: 

A Completed School Health Information and Consent Form for the 2024-26 School Year that is Signed by Parents

Updated Immunizations: Your child needs the following:

           Meningitis Vaccine, T-Dap Vaccine, HPV (optional)

A MSDE Medication Administration Authorization Form (If Applicable) This form is used for any prescription medications that your child may need at school. This form is also used to give any Over-The-Counter Medications such as Acetaminophen or Ibuprofen. Medication should be sent in for your child in a labeled prescription container or small OTC Medication Bottle.


Grade Eight:

A Completed School Health Information and Consent for the 2024-25 School Year- Form that is Signed by Parents
