REQUIRED FORMS FOR ATTENDING
The forms gives youth participants permission from a parent/guardian to participate in Box City. We collect information about the participant, family contacts, insurance provider and health information in case of a medical emergency.
The Covenant for good participation is included in this PDF. All forms are due at the Box City welcome table.
In addition to being an educational and life-changing experience, Box City is a fundraiser. Each participant must have at least 2 sponsors in order to participate in Box City. We encourage all participants to seek gifts beyond your immediate family. Ask neighbors and church members and friends in the community. All funds raised will go to support the chosen beneficiary of this event. Go to the Donate page for more information.
