Registration

In order to register your child in our Religious Education Program, you must be a REGISTERED PARISHIONER with a Parish ID Number (Envelope Number). If you have not registered with the Parish, kindly either go to the Rectory located at 110 Nelson Avenue or call them at 718-983-7873 to make arrangements to register.

• Please follow these directions to complete the form: 

• Download the attached form

• Print the form out

• Fill out all information and sign the 24-25 New Student Registration Form

• Fill out the Media Release Form

• Scan the  24-25 New Student Registration Form, the Media Form, and a copy of the child's Baptism Certificate and 

    email it to aupton@stclaresi.com

• Note: Kindly scan a copy, or take a picture with your cell phone, of your child's Baptism Certificate along with the Registratoion Form and email it to the address above

• If you have a child who is currently in our 1st, 2nd, 3rd, 4th, 5th, or 6th grade Religious Education, you need to complete a Re-registration form for the upcoming year.

• Please click here for the Re-registration form to re-register your returning student to continue his/her religious education and continued preparation for Confirmation.

Please note: 

• • If you have an incoming 1st grade student in your family, please include the name of your child on the form, and also fill out the New Student Registration form in the section above. You must also provide a copy of his/her Baptismal Certificate.

  • Your child/children must register for Religious Education each new school year.
