Current Primary Care Fund Grant Recipients
Agencies currently receiving funds from the Primary Care Fund Program may use thier 2025-26 Primary Care Fund application and supporting documents to apply for the Provider Stabilization Fund. These agencies can forgo filling out the required document by completing the attestaion form (below) and uploading it to thier agency folder.
Current Primary Care Fund grant recipients not wishing to use their 2025-26 application will need to follow the process as outlined under the Applicant Agencies section.
