CMS is looking for additional Medicare-certified hospice providers to test the Hospice Outcomes & Patient Evaluation assessment instrument. CMS plans to collect data from August through this fall.

In preparation for the Internet Quality Improvement and Evaluation System (iQIES) Survey & Certification (S&C) Hospice functionality release, users will need to create two logins:
A HCQIS Access Roles and Profile (HARP) account and a Quality, Safety and Education Portal (QSEP) User Role will also need to be created to access iQIES tutorial videos, located on the secured side of QSEP. Instructions on how to obtain an iQIES user role will come at a later date.
HARP Instructions
Users will need to sign up for an account in the HCQIS Access Roles and Profile (HARP) system at: https://harp.cms.gov/. HARP uses Experian to remotely verify a user’s identity by applying the data that a user provides, such as date of birth and Social Security number, to generate a list of personal questions for the user to answer to verify their identity.
Note: If you receive an error message that your email address already exists when attempting to register in HARP means you have completed some level of identity proofing in the past and that you have an Enterprise Identity Management (EIDM) account. If this is the case, you will need to log in to HARP using your EIDM login information. If you don’t remember your login information, you will need to contact the QualityNet Help Desk at qnetsupport@hcqis.org or 866-288-8912.
After you register or log in to the HARP system and are already an EIDM user, you will be directed to set up two-factor authentication.
Note: If you are an EIDM user, you will then need to log in to HARP a second time; this will ensure that you are able to access QSEP and in the future, iQIES, to request your roles.

QSEP User Role Instructions
If this is your first time logging into QSEP, please follow the instructions below to request access to QSEP. Please watch the following training on requesting access to QSEP: QSEP User Training.
Login into QSEP at https://qsep.cms.gov with your HARP credentials and perform two-factor authentication.
If you had a prior Integrated Surveyor Training Website (ISTW) account, enter your ISTW User ID (i.e., your Surveyor ID number) and click Sign-up.
If you did not have a prior ISTW account, please:
Select your agency from the Agency dropdown.
Select your user role from the User Role dropdown.
Indicate if you are a Security Official or not.
Enter your Surveyor ID number if you are a surveyor. If not, leave the Surveyor ID field empty.
Select the Request Access button.
Await approval notification from the Security Official of your agency.
