Test Your Knowledge Questions: 

1) Is it true that an active wound infection with MDRO and receiving antibiotics cannot be coded under isolation?

2) Did all September assessments have to be completed and submitted by October 1, 2023, due to MDS changes?

3) Is CMS doing away with the 672 completely and replacing it with the 671? 

4) When do we open the functional GG assessment?

6) A Medicare A resident entered the facility on 9/8. A combination MDS ARD was set for 9/15/23 (day eight), and MDS signature, CAA's, and CP all signed the assessment on 9/22/23 (day 15). After submission, our system is showing we are out of compliance for CAAs/MDS based on signature/completion date. Do I tell billing to charge as though we are 1 day out of compliance?

For section N, can the anti-depressant be checked in the first column for classification, but should the second column be checked since there is an indication documented for the medication, or is the second column not checked because it is NOT being used according to the drug classification?

13) Just clarifying that therapy days/minutes are only counted on 5-day MDSs? 

14)  A resident had a quarterly due on 10/3/2023. Due to a decline, it was marked this an SCSA. The resident was enrolled in hospice on 10/5/2023. Does another SCSA need to be completed since hospice was added or will the SCSA completed on 10/3/2023 cover the hospice enrollment? 

15) On an OBRA reentry, is an Admission assessment done each time, or an SCSA if the resident returns and it has been determined by the IDT that the requirements for an SCSA have been met.

18) A resident was admitted on 9/28/2023 with Part A, Tracheostomy/Suctioning. The resident was not seen by MDS and therapy was not provided. The resident died in the facility (DIF) on 10/1/2023. Should a 5-day be completed or just an entry and DIF for MDS purposes?

19) Clarification for dashing admission performance items and setting one goal in Section GG with the Oct 1 MDS changes. There is no longer an SNF QRP requirement to establish discharge goals on the 5-Day PPS assessment. SNFs are no longer required to report at least one self-care or mobility discharge goal on MDSs beginning with assessments with a target date of Oct 1, 2023, and SNFs can ignore the AP warning and proceed with assessment submission. The question relates to dashes on the MDS in Section GG. Do these changes allow for dashes to be entered on Section GG admission/discharge performance, or is there a penalty for this?
