A decline by two levels in any combination when compared to the prior assessment (first assessment at least 46 days prior) will trigger the measure. Assuming they're a 'long stay' resident, of course. Walk 10ft is the new 'Locomotion on Unit' for movement independently worsened. This works the same as the ADL QM, but just one level of decline will trigger the measure.


3) The activity not attempted code “10” is used when there is no equipment available, or the weather is too bad to attempt the task. Code 10 should not be used because the task was not assessed when it could have potentially been assessed for that resident.
