iQIES Survey & Certification Known Issues Log Available
The Known Issues Log is a resource to help you stay informed on iQIES system issues about Survey & Certification (S&C) functionality. It’s a repository of known issues that are updated as issues are reported and resolved. The latest date of a reported issue in the log’s current version is February 9, 2023, with a report date of March 23, 2023.
You can access this log on the secure login area of the QIES Technical Support Office (QTSO) website, where S&C system issues are only viewable by State Survey Agencies and CMS staff.
To view the contents in QTSO, CMS staff, and State Survey Agency users need QTSO access through HARP. To request access:
Log into HARP
Click the “Request a Role in HARP”
Select QTSO for the program and organization
Select QTSO CMS/State User Role
There’s also a Known Issues Log on iQIES.cms.gov, but it’s for issues related to Patient Assessment submission and reporting only; it doesn’t address S&C issues.

LTCF Annual Survey
New! 2023 LTCF Component Annual Facility Survey Review
The National Healthcare Safety Network (NHSN) Annual Facility survey for long-term care facilities (LTCFs) is required for facilities that are currently or plan to report healthcare-associated infections (urinary tract infections), laboratory-identified events for C. difficile and/or multidrug-resistant organisms, and/or prevention process measures by March 1st or prior to reporting in the non-COVID-19 module(s) if beginning after March 1st.
