1) The actual date the resident died should be entered as the death date on the Death in Facility tracking form. The date the mortuary picked up the body is not a factor for MDS coding.

2)  No, the vaccine received in January would have been the 2023-24 dose.  What is up to date now is the brand-new vaccine for 2024-2025 that started to become available in September.

3) The resident would not be up to date. See RAI User's Manual, page 0-20.
