What is Secondary Transition?
Secondary transition services are coordinated activities designed to help students with disabilities successfully move from high school to adult life. This may include education, employment, independent living, and community participation. These services are required by the Individuals with Disabilities Education Act (IDEA) and are tailored to each student's strengths, preferences, and interests. Planning usually begins by age 16 (or earlier, depending on the state) and becomes part of the student’s Individualized Education Program (IEP).

Use interest inventories and skill assessments to understand students’ strengths, interests, and career and educational goals.
Develop post-secondary goals in the areas of education, employment, and independent living.
Design a transition plan that includes services that help students move towards their goals such as courses of study & services/activities.
Establish a transition team to include outside agencies to support students’ post-secondary goals
Monitor progress and adjust the transition plan based on data
