Event Agreement Waiver

Below is the Event Agreement Waiver students AND parents signed electronically upon purchasing a ticket

Failure to follow the rules and regulations will result in the consequences listed below.

North Hills High School Events

 

1.     Students and Parents/Guardians must sign the completed event agreement and permission form for themselves and their guest before tickets will be released to the North Hills student. No ticket(s) will be released without the permission form completed in its entirety by both the student, parent/guardian, outside guest (if applicable), outside guest parent/guardian (if applicable) and outside guest principal (if applicable).

 

2.     All tickets will be sold in advance of the event online. Tickets will not be sold at the door.

 

3.     Students will not be permitted to enter the event without a ticket. All tickets will be scanned at the door via electronic device or students may print out a paper copy of the electronic ticket.

 

4.     No refunds will be given. Tickets are non-transferrable

 

Requirements & Expectations during North Hills High School Events

 

1.     Students must enter events within 30 minutes of the start time and present their valid school ID.

 

2.     Students must stay at the event until dismissed by Administration.

 

3.     Students attending the event will be closely monitored by school administration, staff, and NH security to ensure student safety. Drugs, alcohol, weapons, tobacco, and any other illegal and/or dangerous contraband are not permitted. Portable breathalyzers will be on-site and may be used. Students suspected of being under the influence of drugs or alcohol or in possession of a weapon will be subject to the applicable policies and procedures of the high school and school district. Local police will also be contacted if necessary.

 

4.     All purses will be searched prior to entry. NO BACKPACKS are permitted. All students will be required to empty their pockets prior to entry.

 

5.     Students are not permitted to bring food, drinks, other liquids and/or prescription/non-prescription medications. Students requiring medication of any kind must contact the Activities office prior to the event.

 

6.     Students who violate any of the above rules/expectations or NHSD Polices (board policies/students-parent handbook procedures) will be subject to discipline as started in the Board Policies & Student-Parent Handbook including loss of student privileges to attend future events.

 

Attire

 

·   Appropriate dress is required – Semi-Formal Wear

·   Shoes are required to be worn at all times


Student ID

·   You must provide a valid Student ID when checking into the event -- New student ID's are given to students grades 9 -11 at the beginning of each school year (post picture day). Seniors or anyone who has lost their student ID should use a previous year's ID

·   If you bring an outside guest, they must also provide a valid ID when checking into the event


 

Conduct: Event – 2023- 2024 Events (Homecoming & Prom)

 

All aspects of the Student Handbook and the District Discipline Policy remind in effect during the event

 

·   Students are not permitted to leave the event prior to the end time without administrative approval and parental consent.

·   Inappropriate or disruptive behavior may result in removal from the event

·   Any student suspected or found smoking/vaping, under the influence, using, or in possession of alcohol or controlled substances will face the following consequences

o   Your parent(s)/guardian(s) will be notified to escort you home

o   You will receive disciplinary consequences in accordance with the Student Handbook and charged under any appliable laws

o   If your parent(s)/guardian(s) are not available to take your home, you will remain under police custody

·   Students serving out-of-school suspension or on Social Probation are not permitted to attend

·   No refunds will be issued for this event

 

 

Disclaimers

·   Please note that student event codes may be modified at any time, and event rules can vary from event to event. In such circumstances, the school will provide clear instructions regarding the expectations for a particular event.

 

COVID-19

 

The undersigned acknowledges that attending or participating in events, activities & athletic programs may include possible exposure to a communicable disease including but the limit to MRSA, influenza, and COVID-19. The undersigned further acknowledges that there are aware of the risks associated with COVID-19 and that certain vulnerable individuals may have greater health risks associated with exposure to COVID-19, including individual with serious underlying health conditions such as, but not limited to: high blood pressure, chronic lung disease, diabetes, asthma, and those whose immune systems that are compromised by chemotherapy for cancer, and other conditions requiring such therapy. While particular recommendations and personal discipline may reduce the risks associated with participating in events, activities and athletics during the COVID-19 pandemic, the risk of serious illness, medical complications, and possible death does exist.

 

WE knowingly and freely assume all such risks, both known and unknown, even if arising from the actions, inactions, and negligence of others, including but not limited to NHSD, and assume full responsibility for our student’s participation in events, activities & athletics during the COVID-19 pandemic. We willingly agree to comply with any and all recommendations put forth by NHSD and by federal, state and local authorities to limit the exposure and spread of COVID-19 and other communicable diseases.

 

By signing this for, the undersigned voluntarily agrees to the following wavier and release of liability. The undersigned agree to accept sole responsibility for any injury, disability, illness, damage, loss, claim, liability or cost and expense of any kind that the undersigned, the student, outside guest, or any of the undersigned’s family member(s) may experience or incur in connection with the Student’s attendance or participation in NHSD events, activities or athletics, and hereby release, covenant not to sue, discharge or hold harmless to ourselves, our heirs, and as a parent or legal guardian for the student named, the NHSD, its Board of Directors, officers, agents, employees, and volunteers from and against any and all of liabilities or demands for personal injury, disability, psychological injury, sickness, damage, loss, death, or claims resulting from personal injury or property damage, of any nature whatsoever which may be incurred by the Student or the undersigned or a member of the undersigned’s family relating to or as a result of their or the Student’s participation in or attendance at events, activities and athletics during the COVID-19 pandemic.
