Final Transcript Request

The final transcript request is an especially important part of this form as most post-secondary schools and colleges require an official final transcript to verify the student’s successful graduation from high school. The mailing of a final transcript is NOT done automatically. It is the student’s responsibility to complete this final transcript request and submit it to the CRC for processing after graduation.
