Academic Concerns - Submit when a student's classroom performance is declining and/or the student is in danger of not earning credit.

This report will be delivered to the student's counselor, grade level administrator and special education teacher of record if applicable.

Behavior Concerns - Submit as documentation for non-disciplinary behaviors. For example, changes in attitude or appearance, and any other inconsistent behavior that you want to document without discipline.

Assistance Requests - Submit all attendance concerns.

This report will be delivered to the attendance dean, student's counselor, grade level administrator and special education teacher of record if applicable.

Infraction Reports - Submit when a student's actions have violated school rules and may be subject to discipline. Please contact the parent/guardian and plan for the student's re-entry to your classroom.
