NON-FMHS GUEST INFO

NON-FMHS GUEST FORMS ARE DUE BY 4.16.2024
Only the FMHS Student bringing a guest can fill out the Google form using their student email address.

LATE FORMS WILL NOT BE ACCEPTED - NO EXCEPTIONS.

No Non-FMHS Guest tickets will be sold at the door UNLESS ALREADY PRE-APPROVED.

Guests must be under 21 years of age on 4/20/2024.

When purchasing a ticket for a Non-FMHS guest, you will be asked to input the guest's identification information. 
If the guest is still in high school, this will include either the driver's license number of the guest (or student ID number), date of birth, and a signed Non-FMHS guest form by the Principal of the school they attend. 
If the guest is in college, this will include a driver's license number of the guest (or student ID number), date of birth, and the phone number to their admissions office. 
If the guest is not currently enrolled in college courses, but is employed, this will include a driver's license number, date of birth, and the phone number to their place of employment. 
If one of these three scenarios does not apply to your guest, please contact Mrs. Janoe for further information at janoec@lisd.net.  

Guests must provide Proof of ID upon entering the prom venue.  The information submitted MUST MATCH THEIR FORM OF IDENTIFICATION.

Flower Mound High School reserves the right to deny admission to any attendee.

Are you a FMHS Early Grad?
You are still able to attend prom even if you graduated  early in December!  No need to fill out a Non-FMHS Guest Form - you are still part of the graduating class of 2024, so you are still one of our "current students"!  We can't wait to see all of our early grads back for a memorable night!

Non-FMHS Guest Form  <<< This must be filled out completely and uploaded digitally to the Google Form (linked below)  in order to request  guest approval.
Non-FMHS Guest Google Form <<< This must be filled out and submitted for AP approval BEFORE you can buy your guest's ticket.  If approved, you will be sent an Approval Code within 48 hours of submitting your Google Form.  YOU MUST WAIT FOR YOUR CODE TO BE EMAILED TO YOU BEFORE PURCHASING A NON-FMHS GUEST TICKET.  Failure to use the code specific to you and your guest may result in a refund of your guest's ticket and denial of their attendance to prom.  Do not share your code with anyone else and only use the code you are given.
