I am required to explain the study to participants in greater detail. Once you read and understand more about the study, I will need you to sign a consent form. Also, since I will be using your spoken voice and story in future learning materials, I need you to sign a release form.

Below, you'll find the following files:

(2) Consent Form

(3) Release Form

Please read (1) and sign (2) and (3). Then, return (2) and (3) to porter@fukujo.ac.jp. Once you sign (2) and (3), I will sign them and send a copy to you.

If you would prefer to hear an explanation of (1) from me, we can arrange an online meeting.
