Purpose Statement: This survey is intended to be used to improve, guide, promote and identify pockets of inclusive practice excellence within schools and districts. This self-assessment can be guided through using a coaching conversation or can be done independently.
Instructions:
Part One, includes indicators that promote Membership of all students within a school or classroom.
Part Two provides indicators for Participation and Learning.
Please begin by completing Part One and indicate if you are assessing a classroom or a school and your school district. After completing Part One you are invited to complete Part Two.
An optional Part Three Faculty Survey and Membership and Participation Indicators have been added at the end.
