Request for Proposals

• Part B. Project Budget Proposal using template below

• Part C. Short Answer Questions detailed in RFP

The Proposal Grant Rubric below will be used to evaluate parts A and B of the grant proposals on a scale of 1-5 provided for each component. Reviewers will score applications independently and confidentially. The final  application score must be 40 out of 50 maximum points or more for an application to be considered for funding.     

1. Read the Request for Proposal document below for information about eligibility criteria, commitments and required grant proposal submission materials.

2. Review the budget proposal template and RFP Rubric below for information about how the proposals will be evaluated.

3. Prepare the narrative questions, budget proposal and short answer questions.

4. Click the 'Apply Now' button below to access the submission form.

5. Complete and submit all materials.
