Create a Form.

• Title the form  “Getting to Know You”.

• Change the theme of the Form.

Add 4 getting to know you questions:

• Short answer

• Checkboxes

• Linear scale

• Date

Add a section with 2 additional questions.

Change the Form to a Quiz 

Be sure the form will collect email addresses of respondents.

Assign point values to each of your questions. 

Add a video to the Quiz.

Send the Form to a colleague for completion.

• View the Form responses.

• Choose a Response Destination and view the data.
