1. Child Care Enrollment and Healthy History
2.Health History and Emergency Care Plan
3.Child Health Report (Physical) if a child is under 5 years of age, the individual(s)
will need a physical upon enrollment signed by a physician
5.Field Trip or Other Off-Premises Activity Notification/Permission Child Care Center Form
6.Alternate arrival/ Release Agreement- Only if your child is going to be walking,
biking, bussing, car pooling, etc. home some times.
7. Please also provide a copy of your child's immunization records
8. Please sign the Acknowledgment of the Summer CKC Handbook
