Particpation Forms:

• Athlete Medical Forms (required every 3 years from exam date) – Web Browser Editable PDF Version | Word Document Version

• Class A Form (for 18 years or older). Required every 3 years from signature date. – Web Browser Editable PDF Version | Word Document Version

• Class A Form (for 17 years or younger). Required every 3 years from signature date OR when volunteer turns 18 if before 3 years – Web browser editable PDF Version | Word Document Version

COVID-19 Forms:

• Communicable Disease Waiver (required one time)  

• COVID Screening/Sign In Sheet Template - Can be used at practices.

• COVID Screening Attestation Form - Can be given to participants to fill out the morning before practice to alleviate screening time 

• Incident Report

• COVID 19 Participant Code of Conduct and Risk Assessment - Not required, but strongly encouraged for all participants to review and sign
