Mrs. Burkey will be presenting a one-day suicide prevention lesson using the Erika’s Lighthouse curriculum during English classes:
Sophomores (English 10): Wednesday, October 7
Seniors (English 12): Friday, October 9
CCP English Seniors: You are welcome to join during one of Miss Rosen’s English classes.

2. Complete each form, making sure ALL sections are correct.
3. Mrs. Kane, the high school secretary, will process your permit.
*Your work permit will NOT be processed if any section is missing or incomplete!
