You will need your student's school ID number in order to complete them. (7 digit "S"number) When you have completed all forms, the athletic trainers will review them, and then the athlete will be eligible for athletics. This process takes time so please get them done as early as possible. You should also receive an email from noreply@rankonesport.com (a possible spam folder).
