If your child needs to have the Chromebook repaired or replaced due
to damage or theft, please complete this form.
* If you purchase insurance after damage or theft occurs, the claim will be denied, and you will be responsible for the repair or replacement of the device.
If your child exchanges their Chromebook in the library for a new one for any reason,
please complete this form to notify us of the new asset tag and serial number.
