Prepare all documents listed below with the appropriate number of copies.
Submit complete set of visa application materials to OIS.
The visa applications will be submitted as a group to the Italian Consulate in late October.
Please type your answers into the application form.
Glued to your visa application. If you don't have a glue stick, use a paperclip, but do not staple.
The real thing. If you do not submit this to the OIS Interim Assistant Dean in person, send it via a courier service such as FedEx or UPS. Your passport will be kept in a locked cabinet until it is taken to the Italian Consulate in Boston. Your visa and passport will be returned to you via FedEx.
Itinerary must show your name, and flight information including date of entry and border of entry. A printed receipt from an online booking is perfect, as long as your arrival city is spelled out; airport codes are not acceptable. The date of arrival in Italy and entry point written on your visa application must match your flight itinerary. The return date on your itinerary will not affect your visa application.
Download the Italian Consulate Visa Checklist for reference; it does not need to be submitted with your application. You'll compile documentation as indicated below.
Notes on the Checklist:
Item #4: You can disregard. HOWEVER, please be sure to list your city and state in item #5 of the visa application form.
Item #6: This is the letter that you will receive from UniversItaly as a result of your Online University of Florence registration.
Item #7: Will be created by Angelo Pisano. You do not need to submit this letter with the rest of your materials.
Item #8: You need to supply the most recent month's bank account statement. This is to prove that you have access to cover your personal expenses while you're in Italy. The recommended amount is about €30 per day (either in regular deposits to a parent's checking account with additional supporting documentation or in a savings account under your name).
Item # 9: You can disregard.
Item #10: This will be created by Angelo Pisano. You do not need to submit this information with the rest of your materials.
Item #11: Money Order. Please make sure to purchase the money order AFTER September 30, 2025 when the new visa fee has been published.
Item #13: Use this template of the Affidavit of Support. Note that this form must be notarized.
The Italian Consulate charges a fee to process visas. The amount of the fee changes quarterly and is determined by the Italian Consulate. The visa applications will be submitted as a group in November.
