• Parent Handbook, Health Inventory, Shot record, Lead screening or Lead test, Emergency Card,Parental Rights to Regulated Child Care Pamphlet, Daily Care Sheet, Topical Care Form, Allergy Action Plan if needed, Asthma Action Plan if needed, Food Allergy Action Plan if needed.

Must be current on all shots or have a Dr.'s appointment card for scheduled shots
