Students must submit the following documents to their HPAC file by March 1 of the year they are seeking review. After completing your 1:1 meeting with Brianna, you will receive the HPAC application & the Google Form to upload the required documents (see each item below). Please download each item as a PDF document; they are fillable for ease of completion. There are several forms that require your signature (digital accepted). Please keep the name of the form the same but add your last name.
When you upload the document to the Google Form, please be mindful of the document type (PDF, image, or Google Sheet). If you have any questions, please reach out to prehealth_advising@sju.edu.
The HPAC Committee will need to review your unofficial transcripts. Please download and print a copy to upload to your Google Form. You can find your Class History by clicking on the three little dots in the upper right hand corner of Degreeworks once you are logged in. The dots are right next to the printer and email icons. From the drop down, choose "Class History", then print that screen. Due to the scaling of the information, we request that when you print the document, you change the "scale" to about 70% so that the information is easily readable on one page.
If you completed coursework outside of SJU, please submit those, too.
This Google Sheet will help you document your experiences and is structured the same as it is in the centralized application services (CAS). If you are planning on applying to both MD & DO programs, you only need to complete one tab, but please keep in mind, there is a difference in character limits between the two.
This form is an opportunity to voluntarily provide information about any University policy that you were deemed to have violated during your time at Saint Joseph’s University or at any other college or university that you have attended. Each professional application you submit will also ask you to provide this information and a background check will be run upon your acceptance to verify this information. This form will be kept confidential with only the HPAC Committee Members having access to the information you provide.
This form is required in order to obtain records from SJU Community Standards regarding any policy violation that has occurred.
Send this document (instructions, evaluation form, & submission information) to your recommenders after you've requested a letter from them. You'll also need to include this waiver which will waive or not waive access to your letters; it is strongly recommended to waive your access to your letters.
There is no "form" for the personal statement, however, there is a place to upload it on your Google Form. It should be neatly typed as a Google Doc.
Although it doesn't need to be your final draft, it should be more than an initial draft. Follow the prompt for your chosen professional school, and please limit your character count to 5300 characters for medical school and 4500 for other health professions (dental, optometry, etc.) as that is the limit on their centralized application services. You can review the prompts and essay information for the various programs here.
