Coaches Notes Forms & Information

All paperwork is now found on http://www.registermyathlete.com/schools/ 

All Athletes and parents must complete all paperwork related to Track by the 2nd day of Practice in order to participate.  THERE WILL BE NO EXCEPTION TO THE RULE!

If you do not have your paperwork finished by the 2nd day of official Practice we are not allowed to allow you to participate and you will be excused until it is completed!

Things to be completed (see forms below)

• Register for Track Participation at: http://www.registermyathlete.com/schools/

• Read Track Guide on registermyathlete.  (You will be held to the standards in the Track Guidelines so please know them!)

• Make sure there is a current physical on file. Turn it in to the Athletic Office. To print the new 2024 Sports Physical Paperwork click here: https://www.uhsaa.org/forms/forma.pdf

• Pay $50 Participation Fee to the High School Finance Office (DO NOT PAY ON REGISTERMYATHLETE.COM)

• Last date to signup for track will be March 18, 2024
