Toronto Central Home at Last

The Toronto Central Home at Last Program ("HAL") helps frail older adults without immediate support to arrive home safely upon discharge from hospital, and receive follow-up services at home.

Eligible seniors leaving hospital are: 55 years or older, without support from friends or family upon discharge, and able to direct their own care.

The Toronto Central Home at Last Program ("HAL") is currently piloting a new program, titled, “HAL 2.0”, which offers clients with additional Transition Coach supports for up to 12 weeks after transitioning home from the hospital with the objective of reducing hospital readmissions. HAL 2.0 is ideal for clients who may require:

• Assistance to book appointments and associated transportation;

• Appointment reminders;

• Medication reminders;

• Safety checks.

HAL is a partnership between hospitals, community support service agencies, and the Toronto Central Local Health Integration Network (TC LHIN) Home and Community Care. HAL is funded by the Toronto Central LHIN through Ontario's Aging at Home Strategy.
