Dr. E. Antonio Mangubat [aka Dr. Tony Mangubat] was a surgeon with Puig Medical Group (PMG) [aka Cleveland Hair Centers] from 1989 – 1996. I am one of the many victims who were left SCARRED and DISFIGURED undergoing procedures by Mangubat while he was a surgeon with PMG. As clearly shown below, Dr. Mangubat was a FULL PARTICIPANT in PMG’s incompetent, unethical, and illegal conduct that is a COMPLETE DISGRACE to the medical profession.
Mangubat has resorted to Outright LYING to try to cover up his CON-ARTIST past.
Outright Lying By Tony Mangubat:
Tony Mangubat claimed in his online resume that he has worked for his clinic in Seattle, La Belle Vie Cosmetic Surgery, from 1987 to the present, but he did not obtain his Washington State Medical License until 1994.
The truth is he didn’t start working at La Belle Vie until he moved back to Seattle in 1995-1996, and he worked for Puig Medical Group (PMG, aka Cleveland Hair Centers) from 1989 – 1996, where he engaged in Fraud and Unethical Conduct. In particular, while with PMG, Mangubat preformed disfiguring “Brandy Scalp Reduction” surgeries that were pioneered by Dr. Dominic Brandy and the subject of malpractice lawsuits, see related news articles:

As clearly shown by the info on the Washington State Government’s website above, Mangubat did not obtain his Washington State medical license until 9/29/1994.
So if Mangubat were working at La Belle Vie since 1987, he would have been doing something Illegal, i.e., practicing medicine in Washington State Without A License from 1987 – 1994!

As I showed above and in Attachment 1 below, Tony Mangubat engaged in Illegal Acts by being Totally Dishonest and Lying on his resume from 2019 – August/September 2024, a clear violation of section 18.130.180 of Washington State Law that applies to all healthcare professions. Mangubat is also still To This Day LYING on his LinkedIn Profile and Violating Washingon State Law, because on his Profile he claims he has worked for his Seattle clinic since 1987 (see below in Attachment 1).
Tony Mangubat has also engaged in other arguably unethical conduct to try to generate new business, and this is not just my opinion, but that of independent doctors/journalists associated with a Medical Journal. The remainder of this webpage documents and proves my allegations, i.e., Tony Mangubat’s Outright Lying and past Fraud and Unethical Conduct. I have provided substantial evidence to back up my claims because the Mangubat family has accused me of being a disgruntled former patient/client of Tony Mangubat’s who is just making wild and untrue allegations.
The evidence below includes several links to independent sources that prove Tony Mangubat’s Outright Lying on his resume to cover up his past Fraud and Unethical Conduct.
Attachment 1 below has detailed documentation and Proof on Tony Mangubat’s Outright Lying on his resume to try to cover up his past. This Proof includes several versions of Mangubat’s resume as HE Posted them himself in past years (2010, 2013, and 2018) in which he proudly stated he was a Physician/Surgeon with Puig Medical Group (PMG, aka Cleveland Hair Centers) from 1989 – 1996.
Attachment 2 below documents and proves Tony Mangubat’s past conduct involving Fraud, False-Advertising, and Totally Unethical Conduct while a Physician/Surgent with Puig Medical Group from 1989 – 1996.
Finally, I did not file a malpractice lawsuit against Tony Mangubat because after spending my savings on botched surgeries performed by Mangubat, I did not have the resources to pay for a malpractice attorney to take my case. Also, I could not find an attorney who would take on my case a contingency fee basis before the three year statute of limitations expired.
ATTACHMENT 1: PROOF OF MANGUBAT OUTRIGHT LYING ON HIS RESUME WITH INDEPENDENT SOURCES
Here is proof that Mangubat was LYING on his online resume from 2019 to August/September, 2024 to cover-up his past fraud while he was with Puig Medical Group (PMG, aka Cleveland Hair Centers) from 1989 – 1996:
1. As stated above, Tony Mangubat did not obtain his Washington State Medical License until 1994. So if Mangubat were working at his Seattle clinic La Belle Vie since 1987, he would have been doing something ILLEGAL, i.e., practicing medicine in Washington State Without A License from 1987 – 1994!
2. Mangubat, on previous versions of his online resume as HE posted them on his clinic’s website, clearly stated he worked for Puig Medical Group (PMG, aka Cleveland Hair Centers) from 1989 – 1996, and not La Belle Vie until 1994. See below for several past resumes posted by Mangubat himself online.
3. A 1996 online Court opinion on a lawsuit between Cleveland Hair Centers and Puig Medical Group (PMG) cites Mangubat as a “Puig Doctor”, i.e. a “Puig Medical Group, S.C. ("Puig Group")” doctor, proof he was employed by PMG in 1996, not La Belle Vie Cosmetic.
The truth, which Mangubat is trying to conceal by Outright Lying on his resume, is that Mangubat worked for PMG from 1989 until 1995-1996 during which time he committed Fraud and Unethical Conduct (see proof in Attachment 2 below), and did not start working as chief surgeon/medical director at his Seattle clinic until late 1995 or 1996.

3. A 1996 Court decision/opinion in a lawsuit between Cleveland Hair Clinic and Puig Medical Group (PMG) (Carlos J. Puig’s business) states Mangubat was a surgeon with PMG. This opinion referred to Tony Mangubat as one of “two Puig Group doctors”.
This opinion even confirms that Tony Mangubat performed the brandy scalp reduction procedure (see Attachment 2 below for details about this disfiguring surgical procedure): “Dr. Mangubat was the only doctor at Rosemont who performed Brandy Flap procedures” (Pages 9 of Court Opinion); “Those doctors include Dr. Mangubat, the Puig Group doctor who performed the Brandy Flap procedure at the Rosemont facility.” (Footnote 17 on Page 38 of Court Opinion).

i) The first sentence states that this lawsuit involves PMG:
“This action by Cleveland Hair Clinic, Inc. ("Cleveland Hair") against Dr. Carlos Puig ("Puig") and Puig Medical Group, S.C. ("Puig Group")":
ii) Page 9 of Court Opinion PDF file, Paragraph 34 clearly states in 1995-1996 Mangubat was a “Puig Group doctor”. The text of Page 9 also confirms Mangubat performed the disfiguring Brandy scalp reduction procedures that generated substantial profits for PMG/Cleveland Hair Centers:

Attachment 2: Proof with Documentation of Tony Mangubat’s past Fraud and Unethical Conduct while a Surgeon with Puig Medical Group (PMG) from 1989 – 1996
As clearly shown below, Dr. Mangubat was a Full Participant in PMG’s incompetent, unethical, and illegal conduct that is a Complete Disgrace to the medical profession. Tony Mangubat Lied to his patients by overpromising and misleading on patients’ cosmetic results. Mangubat’s motivation was GREED, i.e., generating new business/clients for PMG, so Mangubat put profits well ahead any ethical obligation to prioritize a patient’s welfare.
A formal Ethics Complaint was filed against Dr. Tony Mangubat with medical organizations he lists on his resume. As shown in Attachment 3 below, an ethics complaint was submitted against Mangubat with the University of Washington (U of W), Medicine. As a result, U of W Medicine required Mangubat to correct his resume to make clear he was not associated with them as of 2009.

According to Dr. Tony Mangubat’s Own resume posted on his website prior to 2019 (see Attachment 1 above), he was a cosmetic surgeon for Puig Medical Group (“PMG”, aka Cleveland Hair Centers) from 1989 – 1996.
PMG's doctors like Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat) and his former boss at PMG Dr. Carlos Puig [aka Dr. Carlos J. Puig] were engaged in Unethical, and Illegal conduct and used high pressure sales and false advertising to lure unsuspecting clients to become their patients (Proof below).
In mid 1990s, Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat ) and PMG promised to cure my premature baldness in my 20s and give me a “full head of hair” [these words are an exact quote of a complete Lie from the PMG cover letter, see below] after the hair transplants and “Brandy” scalp reduction surgeries. Instead, Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat ) left my head Completely Scarred And Disfigured with only a few hairs growing that were of Very Little, if any, cosmetic significance! I have had to have At Least 10 reconstructive cosmetic surgeries to reverse what Dr. Tony Mangubat did to me.
I describe below on this webpage the “Brandy” scalp area reduction surgery that Tony Mangubat performed on me. This procedure is Major Surgery that leaves the head with Large Scars around the scalp area that is removed. In my case, Dr. Tony Mangubat performed a few “Brandy” scalp reduction procedures that left a Huge horse-shoe shaped 8+ inch long and ½ inch thick Scar from one Temple to the Crown and back down to the other Temple on my head.
Tony Mangubat, Carlos Puig, and Steven Hopping wrote a medical article on this scalp reduction procedure. In this article, you can Clearly see the Huge Scar left by the surgical procedure, something that was Never Disclosed to patients by PMG (see below). I was forced to undergo multiple Scar Revision surgeries to make the Scars Mangubat left on my head smaller.
I became a patient of PMG in the mid 1990s and I received the 1994 PMG brochure and letters filled with Lies, Hard Sell Tactics, False Advertising and Deceptive Language. Below is the “cover letter” I received from Puig Medical Group’s (PMG) [aka Cleveland Hair Centers] in 1994:
The most Blatant and Completely 100% LIE in PMG’s Cover Letter is when in the cover letter, PMG promised the potential client that PMG’s hair transplants will result in a “full head of hair” [2nd line of 2nd para of letter] that is "virtually non-detectible." [5th bullet in letter] No Half-competent hair transplant doctor would ever say this because of the limited amount of donor hair in the back of the head of a typical hair loss patient, such as me, which Limits the number of hairs that can be transplanted to balding areas. But PMG was perfectly willing to say “a full head of hair” to Maximize its sales and Lure unsuspecting clients into selecting PMG instead of one of its competitors.
If Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat ) & Dr. Carlos Puig [aka Dr. Carlos J. Puig] were Half-honest with their patients, they would have Never claimed to restore a “full head of hair” in the PMG’s cover letter to me and other potential clients.

Please note that on Page 1, the part underlined in red clearly shows that Dr. Tony Mangubat was a doctor with Puig Medical Group (PMG) from 1989 until the date of the resume (i.e. mid-1990s). Please note the text at the bottom [underlined in red], which Clearly shows that Dr. Mangubat performed “Brandy”-style scalp surgeries that left me Scarred and Disfigured. “Brandy” refers to Dr. Dominic Brandy, who was in 1996 Sued and Paid damages for performing such scalp reduction procedures, see links to news articles about the Brandy lawsuit:

Please note that at the bottom of page 2, it clearly says [underlined in red] that Dr. Mangubat performed “71 Modified Brandy Flaps”, and he did a few "Brady" scalp reduction procedures on me.
As I mentioned above, the “Brandy” scalp area reduction is Major Surgery that leaves the head with Large Scars around the scalp area that is removed. I was forced to undergo multiple Scar Revision surgeries to make the Scars Mangubat left on my head smaller.
Below are the cover page & page 1 from PMG's 1994 brochure that has Extremely Misleading Statements and Lies:
- E.g. Outright Lies: “Advances in surgical techniques now allow for complete and permanent correction of this cosmetic imperfection.” [Page 1 of PMG booklet, see above] A Complete Lie: even in 2024 (30 YEARS after the date of PMG’s 1994 brochure), advances in surgical techniques do Not [even in 2024] allow for a complete correction of baldness (i.e. a “full head of hair” as claimed by PMG”) for many hair loss patients. The key challenge is limited donor hair in the back of the head, which limits the number of hairs that can be transplanted to balding areas, and any Half-competent, ethical doctor would know this and never make this claim (i.e. of “complete correction” made by PMG).
- E.g. of Hard Sell tactics: “Let's face it: baldness suggests premature aging.” [Page 1 of PMG booklet]. Male pattern baldness is due to a person having genes programmed to result in hair loss, and that’s it; baldness has Nothing to do with premature aging!
- At the bottom of Page 1 it states that the purpose of this booklet is to give patients "all the information" a patient would need regarding hair transplants, and that the staff is not going to "try to sell" them on the surgery. Baloney! As Proven below (see page 8 below), this booklet was created to sell surgeries, Not "educate patients".
Below is page 7 of the 1994 booklet that contains LIES based on what is Omitted from the text.
-Another E.g. of Outright Lies, “Bald Area Reduction … can eliminate a major portion of the bald area.” [Page 7 of PMG’s 1994 booklet] This is a Lie because of what is Omitted from this claim. As I mentioned above, bald area reduction is Major Surgery that leaves the head with Large Scars around the scalp area that is removed. In my case, Dr. Tony Mangubat performed a few “Brandy” scalp reduction procedures that left a Huge horse-shoe shaped 8+ inch long and ½ inch thick Scar on my head! But on Page 7 of the PMG brochure, there is No mention of a Scar and the photo of the patient shows no Scar anywhere.
Again, Dominic Brandy lost a malpractice lawsuit for performing these “Brandy” scalp surgeries:

But PMG in its marketing brochure makes No Mention of any scarring!! Talk about doctors (PMG doctors like Dr. Tony Mangubat & Dr. Carlos Puig &) who leave out Critical info needed for a patient to make an informed decision about the major surgery!
Above is Page 8 of the PMG booklet which showed an illustration of a patient going from almost complete baldness at his consultation, to having a full head of hair, with the PMG asserting "final result eight months to one year later". This is Completely Misleading since nobody can realistically expect to go from almost complete baldness to a full head of hair in only eight months to a year.
Hair transplants Cannot restore a full head of hair on a man with extensive baldness, yet this "sample patient" has restored everything, including his crown. Hair transplants are limited to the amount of Donor Hair a person has, and showing a final result of a full head of hair is AN Outright Lie & Totally Unethical. The graphics on this page Prove that PMG’s intent was to Mislead and Lie because ALL PMG saw in the patient was DOLLAR SIGNS and profits!! PMG’s doctors didn’t care in the least about the patient making an informed decision.
The booklet also claims that virtually anybody can get a hair transplant, and that the biggest factor in a hair transplant being a success is the experience of the clinic (and not the characteristics of the patient himself). This is also misleading. The unique characteristics of the patient are equally important to the level of success they can expect. It is important that a prospective patient is told right away that different patients will have widely varying levels of success. But that's not so good for business, so that is kept "hush-hush" here.
Then it is stated "You know you have a problem or you wouldn't have requested this material". HERE PMG IS STOOPING TO HUMILIATION TO TRY TO “CLOSE’ THE SALE!!” Would a legitimate physician tell a self-conscious patient that their hair loss is a problem?? Of course Not!! Maybe the balding men who don't get cosmetic surgery "don't know they have a problem"?!
Didn't this booklet begin by claiming that the clinic would not try to pressure anyone into a hair transplant? Clearly the Main Point of this booklet was to Say and Do Whatever It Takes (Pressure, Mislead, Lie), no matter how Unethical, to close the sale for PMG.
Below is the insert page from the 1994 PMG booklet that has more Lies.
- E.g. of Misleading statement/Lie: “The most natural hairline that is virtually undetectable...Cosmetically incredible….You will look and feel the way you hoped to Again!!” [Insert page above of PMG booklet]…in the case of most patients, including me, another Complete Lie. My hairline was anything BUT natural after Dr. Mangubat's procedures with a few hairs growing in different parts of the hairline and the majority of the front hairline with no hair at all!! With the Limited amount of donor hair and hair loss on the front and crown in many patients, there is No Way there would be enough donor hair for hair transplants to result in a "full head of hair" with a "virtually undetectable" hairline!!
So WHY was the Puig Medical Group (PMG) creating this false premise in the minds of potential patients? Because building False Hopes in patients is good for Boosting Sales. Tony Mangubat (aka Dr. E. Antonio Mangubat) was part of the team at PMG that Only cared about SALES, and patient’s welfare was Not a main concern. That is why doctors at PMG (e.g. Dr. Tony Mangubat & Dr. Carlos Puig) were willing to say Anything, Even Outright Lies, to ensure that the potential client would select PMG, and not one of its competitors, for hair transplants.
No One has to just rely on my review to decide that Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat) and Dr. Carlos Puig [aka Dr. Carlos J. Puig] were engaged in Fraud and Unethical Conduct. Antonio Mangubat ) himself on his online resume Proudly said that he worked as a cosmetic surgeon at PMG from 1989 – 1996 (See Attachment 1 above), and he was a Full Participant in PMG’s incompetent, unethical, and illegal conduct that is a Complete Disgrace to the medical profession. I know this Firsthand because of what Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat ) did to me, i.e. promised me a “full head of hair”, and left me Scarred and Disfigured!!
Since 2019, Dr. Tony Mangubat (aka Dr. E. Antonio Mangubat ) has decided to Lie on his resume and delete the reference to Puig Medical Group from his official resume on his website, La Belle Vie Cosmetic Surgery Centers.Until August/September, 2024 Mangubat Outright Lied by saying he worked at La Belle Vie since 1987, Even Though he did not have permission (i.e. a license) to practice medicine in Washington State before 1994. After I pointed out Mangubat’s Lying, now Tony Mangubat has decided to just NOT POST his resume online to try to hide from his Con-Artist and Scamming past!

Attachment 3: Ethics Complaint Against E. Antonio Mangubat (aka Tony Mangubat)
An Ethics Complaint was filed against Dr. Tony Mangubat with medical organizations he lists on his resume. In 2012, an Ethics Complaint was submitted against Mangubat with the University of Washington (U of W), Medicine. As a result, U of W Medicine required Mangubat to correct his resume to make clear he was not associated with them as of 2009.
In 2012, prior to the ethics complaint, Mangubat stated on his resume that he was associated with Univ. of Washington Harborview Medicinal Center (“UofW Med) from “2004 – Present”.
BUT, based on the Ethics Complaint against him, UofW Med required Dr. Mangubat to Correct his resume and Make Clear he was not associated with UofW Med after 2009.
