Forms

The Ottawa County Dispatch Authorization and applicable single or two parent agreement form must be entirely filled out and returned to Grand Haven Township Fire Rescue two weeks prior to the first day of the academy.  These forms will be emailed to those signed up in May along with additional information. The completed forms can be scanned into a PDF and emailed to dpeterson@ghtmi.gov, dropped off, or mailed to:
