Athlete Registration Form Please fill out an application for each child. This form is due by 4/6/2021 and is available to print at the bottom of this page.

Athlete T-Shirt (https://stjoesephtr21.itemorder.com/ ) Cost of t-shirts is $10. Each student must have a team short sleeve, as this is their uniform. Unlike years past, shirt orders MUST be placed through the online store.

In addition, there is a long sleeve, and two sweatshirts options available as extras. Friends and family are welcome and encouraged to order through this same link.

*Orders will be submitted for printing on April, 6th.

Athlete Physical Exams

If your child has not competed in a sport they will need a physical exam, use the physical exam permit card document. Otherwise, use the alternate year athletic permit card, which does not require a physical exam. These forms are due by 4/6 and are available through the office.
