Click on the file name at the bottom of the page to open the file and then print the form. Our annual membership fees are:
Individual $15.00 USD
Couple $25.00 USD
Senior $10.00 USD (if your 65th birthday has occurred before the start of the membership year)
Our membership year goes from October 1 to September 30.
At this time, we cannot take payment electronically (e.g. Paypal) or by credit credit card. We also do not accept payment for multiple year memberships at this time. If you are applying from a country other than the United States, the annual fee is an additional $3 (e.g. $18.00 USD for individual).
SCAN is authorized to do business in Connecticut in accordance with Section 7-51 of the General Statutes. Our membership cards allow access to vital records in towns and cities in Connecticut.
