The Sponsor of this Applicant must be a member of the Society for at least three years
and he must be in good standing at the time of this application.
In applying for membership to the Societa' Santa Maria Maddalena, I agree to abide by all rules and regulations and to uphold the integrity of the Societa' at all times, I also agree to promptly pay any dues or assessments that are established therein or in the future.
I understand that the dues are $35.00 per year, plus a $5.00 initiation fee, that initially my dues will be pro-rated at $3.00 per month based on the Month that I join and calculated to the end of the calendar year.
