The adverse events being measured were medication error, pressure injury and falls. The study concluded that the more patients a nurse had the chances of one of these adverse events from happening increased. The purpose of the study was to measure inadequate communication in relation to errors on admission, during hospital stay and after discharge. This article is relevant because it increases awareness of communication barriers and how they affect patient safety. The results of this article emphasize on having an accurate patient medication list from admission through discharge. Most of the errors were in correlation with the patient’s medication list. Nurses can use this as a tool, not only to increases awareness, but to also educate their patients prior to discharge on their medications.
