Two bug fixes:

- Custom poll was broken - length byte is stuck at 4. Bug since 167.2. Advanced custom poll was unaffected

- In EGMMODE, QSIM's response to a Cash-out Poll was hard-coded to go into a cancel credit. Changed QSIM to do nothing in response to a Cash-out poll in order to allow the user to decide what to do when they see the cash-out poll event appear in QSIM EGMMODE.
