CME Forms, Policy, and Tools

Educational activities certified by the Center for Continuing Medical Education currently include live courses, live regularly scheduled series (RSS), and Internet-based activities (live and enduring). Educational techniques and strategies employ a variety of interactive and innovative methods, including case presentations, self-assessment tools, simulation/hands on workshops, and small group discussions. As the design, method, and format is based on the educational needs and learning objectives, it is expected that educational activities certified by the University of Chicago Center for Continuing Medical Education will lead to improved physician performance, patient health, and community health outcomes.

Please use the forms and documents below to help plan your educational activity. Applications for CME certification only are due 90 days prior to the activity start date. If grant submissions are being sought, it is highly recommended to apply at least 12 to 9 months prior to the start date of the activity.
