Forms for admission to Prakash Classes can be downloded from the following links or you can obtain from the Director by paying Rs 5/-in cash.

The same Form should be submitted before joining to the Classes. If you do not wish to submit the form with required fee or you fail to do so ,you will not be permitted to sit on the third day until you do so.
