Medication info

Pursuant to district policy for administering medication to students:

1. Parent/Guardian must completely fill out one Medication Administration Log for each medication your student takes. Please send your child to Kim McGrath Anderson to pick up the form and bring log and medication to school by this Friday. No medication (prescription, non-prescription, herbal) will be allowed on trip without this form.

2. Parent/Guardian shall be responsible for providing all medication to be administered. No medication shall be taken from the clinic; it must be provided specifically for this trip following the guidelines below:

-prescription medication MUST be provided in an individual pharmacy labeled container.

-label MUST state student's name, name of medication, how often it is to be administered, the dosage, when medication is to be discontinued (where appropriate), current dates, and the name of the authorized prescribing healthcare provider.

-Injections of medications may be administered only when a student has a condition that is potentially life threatening specifically identified and planned for by the authorized prescribing physician.

-only pack exactly the amount needed for this trip plus 2 in case accidentally dropped.

-Non-prescription and herbal medicines must be packed in sealed, original packaging clearly labeled with name of student.

-UPDATED FROM DISTRICT TODAY: Parent/Guardian will also need to download print, and sign this Medication self carry form if student will self-carry, and self-administer any medication (prescription, non-prescription, herbal). You can also have your student pick up this form from any PAH teacher.

Items for comfort during the flights. Place in Carry-on—ONE per person only

1. Sweater or Jacket ---planes can get cold on flights

2. Noise reducing headphones-earplugs

3. Device for videos-videos-music

4. Snack—yes you do get snacks and refreshments. Due to length of flights time between meals are extended. Be Prepared.
