You will create a survey based on the topic you developed in class. Next, you and Mrs. Planchon will give the survey to students. Finally, you will graph the results of the survey and write an analysis of the results.
Create You Survey
To create your survey, please login to your Google Drive.
Click New > More > Google Forms
Step 1: Name your form
Click where is stays Untitled Form in the top let corner and type a name for your form.
Step 2: Type in your first question
Question title: Type in the question you want to ask
Help text: You can leave it blank, or type something here to help your reader understand the question.
Question type: Pick Multiple Choice
For example, if you want to know if the person likes red, yellow:
where it says Option 1 type Red
where it says Option 2 type Blue
where it says Option 3 type Yellow
If your answer choices are yes or no, type Yes for option 1 and no for option 2
Click the blue Done button under the question when you are done
Step 3: Type in the next question(s)
To add a question, lick the Add Item button under the question you just completed. Repeat Step 2.
Step 4: Choose a theme for your form
Click Change Theme underneath your toolbar at the top. A window will open on the right side of your screen with available themes. Click on one you would like.
Step 5: View Live Form
Click on View Live Form underneath the toolbar at the top of your page to see how your form will look to your survey respondents. A new window will open. To edit your form, click on the tab with your form you were working on.
Step 6: Send your form to Mrs. Planchon
Click on the blue Send Form button in the top right hand corner of the page. Click under Send Form via Email and copy and paste in Mrs. Planchon's email address: Tiffany-Planchon@scusd.edu. Mrs. Planchon will have another class answer your survey quesitons.
Step 7: Print your form
Click on View Live Form. Right click on the form and select Print.
Step 8: Set up a response table for your survey
To record responses to your survey that you give to your friends, create a new document.
Title your Document by clicking on Untitled Document and giving it a name
Write the name of your survey and your name at the top of the page
Insert a table
Click table > Insert a Table
Your table should have the same number of columns as the number of questions in your survey. Add about 20 rows.
Print out your sheet to the lab printer.
