Use this application for spring and fall.

This application is in two formats. PDF and MS Word. 

Please use the pdf version if possible (name says 'application for 2026.pdf'). 

The other version, MS Word format (name says 'application for 2026.docx'), is greatly affected by differences in software versions (like Internet Explorer) and may not format correctly on your computer. 

Please click on the pop-out in the top right corner to open and then select print. If you are not able to print , please send an email to joycesprik3@gmail.com. Thank you.

 Paper copies of the application will also be available at the Roche Community Center, Sebastian's Barber Shop in Roslindale,  and Phil's Barber Shop in West Roxbury.
