Use this application for spring and fall.

This application is in two formats. PDF and MS Word. 

Please use the pdf version if possible (name says 'application for 2025.pdf'). 

The other version, MS Word format (name says 'application for 2025.docx'), is greatly affected by differences in software versions (like Internet Explorer) and may not format correctly on your computer.

 Paper copies of the application will also be available at the Roche Community Center, Sebastian's Barber Shop in Roslindale, Elie's Barber Shop in West Roxbury and Phil's Barber Shop in West Roxbury.
