Kaitlin is an Advanced Clinical Pharmacist at Brigham and Women’s Primary Care, South Huntington. In her current role, she manages patient's with diabetes, hypertension and hyperlipidemia under CDTM protocols. Her interests include transitions of care, patient engagement, and population health management.
Ashley is a Manager for the Clinical Pharmacy department and PGY1 Residency Program at Greater Lawrence Family Health Center. She practices under a CDTM agreement to provide patient care. Ashley's interests include chronic disease management, preceptorship, and community outreach.
Jackie is a Senior Clinical Pharmacist at Atrius Health in Boston, MA. In her current role, she is also the Program Manager for the Clinical Pharmacy Clinics and oversees the Clinical Pharmacy Credentialing Committee. Jenn's areas of interest include chronic disease management, collaborative practice, population health management, and ambulatory pharmacy advocacy.
Hailey is an Associate Professor of Pharmacy Practice at Massachusetts College of Pharmacy and Health Sciences. She is an ambulatory care pharmacy specialist with the Board Certification in Ambulatory Care Pharmacy and a Certified Diabetes Care and Education Specialist. She practices at the Mount Auburn Hospital Primary Care Center in Cambridge, providing medication therapy management and chronic disease management focusing on diabetes and obesity while providing experiential education to pharmacy students and pharmacy residents.
Madeleine (she/they) is an ambulatory pharmacist at Sturdy Health in southern MA. She completed her PharmD at the University of Michigan, followed by a 2-year residency in ambulatory care with a focus on pharmaceutical care leadership at the University of Minnesota. Madeleine is the first ambulatory pharmacist in their health system, and they have a collaborative practice agreement with physicians across the 6 primary care sites in Sturdy. Her areas of interest include comprehensive medication management, population health, and expanding pharmacist services.
Jen is a Professor of Pharmacy Practice, at the Massachusetts College of Pharmacy and health sciences in Boston. She developed and directs the cardiometabolic services at Well Life Medical in Peabody, MA where she is a clinical pharmacist and certified diabetes care and education specialist. She is board certified in advanced diabetes management and a fellow of the American College of Clinical Pharmacy. She's been practicing for over 35 years and has numerous publications in the areas of diabetes and cardiometabolic disorders.
Dr. Kelsey Norman currently serves as a clinical pharmacy coordinator in the Cardiovascular Center at Boston Medical Center where she provides care for patients with for hypertension, hyperlipidemia, and heart failure amongst other cardiovascular diseases. In addition, Dr. Norman also serves as the PGY2 Ambulatory Care Pharmacy Residency Program Director where she oversees the training and education of the pharmacy departments' two PGY2 Ambulatory Care Pharmacy Residents. Dr. Norman is board certified in cardiology and ambulatory care pharmacy. Dr. Norman received her PharmD from the University of Rhode Island. Following graduation from pharmacy school, she completed her PGY1 Pharmacy Residency and PGY2 Cardiology Pharmacy Residency at VA Ann Arbor Healthcare System in Ann Arbor, MI
Amanda is a PGY1 Pharmacy Resident at VA Boston Healthcare System and a recent graduate of Northeastern University. Her areas of interest include primary care, anticoagulation, and heart failure. In her current roll, she is training to become an ambulatory care pharmacist, managing patients with chronic disease states and obesity. In her free time, she enjoys spending time outdoors and exploring new places and foods.
Peter Hiller is a PGY1 Pharmacy Resident at MCPHS/Atrius Health and a recent graduate of the University of South Carolina College of Pharmacy. Originally from the Chicagoland area, Peter’s professional interests include ambulatory care, academia, and the application of artificial intelligence in pharmacy practice. In his current role, he is training to become a clinical pharmacist, managing patients with chronic diseases. Additionally, Peter serves as an adjunct instructor in pharmacy practice at MCPHS University, where he teaches practice labs and facilitates seminars for PharmD students.
Gabriella is currently a PGY-2 Ambulatory Care resident at Boston Medical Center. She completed her PGY-1 residency at VA Boston Healthcare System, and attended pharmacy school at Northeastern University. Her practice interests include geriatrics, primary care, and complex disease management. Outside of work, Gabriella enjoys boxing and playing Nintendo Switch.
