All students must read this and take this home.

Safety Contract: (Attached Below)

All students must have on file with District 215 a Science Safety Contract signed by both a parent and the student.

A second copy of the safety contract must be in the students folder at all times.

Class Supplies and Information Sheet: (Attached Below)

All students must return the bottom portion completed and signed by a parent.

Student Information Sheet: (Attached Below)

Students will fill this out in class August 21st.

All students arriving after August 21 must fill this out and return it by email to the instructor.

Save the document with your name and class period as the document file name. Send it to the instructor as an attachment in an email from your student email account. Your Last Name, First Name and Class Period must be in the subject line.
