Transcripts & Other Forms

College Transcripts - Seniors must use Parchment/Docufide to request all transcripts and mid-year transcripts. Exception 1- If you are applying to a Common App School, make sure that you compete the education section with your schedule for both semesters first and then request me as the school official. I will receive an email notification from Common App to attach your transcript (do not request on Parchment for Common App). If you need a Mid-year report and transcript for Common App schools, send me an email requesting this. Exception 2-If your school is not affiliated with Common App, Parchment, SendEDU, or other electronic web service, then you may use the transcript request form below and attach $2.00. Give this form to Mrs. Elliott or Ms. Carla Williams (secretary) in the main office at least 36 hours before pick up or mailing (no last minute requests).

Important - If there is a school official or counselor portion to fill out or sign, include that form with your transcript request form and $2.00. We will need at least two weeks to process your request if the extra school official or counselor portion is needed. Give all forms to Mrs. Elliott or Ms. Carla Williams (secretary) in the main office. (no last minute requests).
