DUE NOW! Each church is strongly advised to submit this form for your group, especially if you will be coming in your church's or privately-owned vehicles. Send this sample form to your church's insurance agent/company so they will word and complete your form correctly. Please have your agent scan/email the completed form to David Flippen at lflippen@younglife.org as early as possible prior to the retreat. NOTE: Each person in your group is also required to complete an online or paper Young Life Guest Consent Release form.

Guest Consent Release Form

Click HERE for the ONLINE Guest Consent Release form. Copy the link and email it to your students' parents and your leaders. IMPORTANT! On the online form under "Please select your group name," scroll up or down and click on "Modgnik MS (Sept 11)." In the "Church Name" box click on the down arrow and select the name of the church you're coming with. If your church is not listed, let Nancy know and she'll ask Rockbridge to add it to the list (nancy@cov-pres.org).

Completed online forms go to the Rockbridge office. A link to a list of who has submitted an online form will be sent to you closer to the retreat date.

This form is REQUIRED by Rockbridge for EVERYONE in your group coming to Modgnik. We aim to have these forms ALL completed online! If you absolutely must bring paper forms, please bring them in alphabetical order to Friday night check-in. Note that this form does not cover you or your church from liability; it only protects Rockbridge from liability.
