If your child needs medicine while in our care, please fill this form out and turn it in to the director along with the medicine. Please remember that all medicine provided must be Dr prescribed, in the original bottle, with label intact. DO NOT send any medicine in your child's back pack or lunch. It must be handing directly to the director. Thanks!
